Eligibility and Terms & Conditions
The primary cardholder must be 18 years of age or older; minors can join as beneficiaries with their guardian's consent.
The card is non-transferable and issued in the name of the Primary user.
Discounts vary by service, hospital, and treatment type; exclusions may apply.
The card is valid until 31st December 2026 and requires renewal for continued benefits.
Only one free consultation with a general physician per card is allowed until the card's validity expires.
No two offers can be combined under any circumstances.
Primary cardholder must show valid ID at registration before availing services.
The submitted address must match the enrolment address. Any expired, unclear, or mismatched documents will cancel membership and require resubmission.
If submitted documents are found mismatched or fraudulent after activation, membership may be suspended or terminated immediately
